Job Description
Under physician's orders, the Care Center Registered Nurse (RN) / Licensed Practical Nurse (LPN) is responsible for identifying and coordinating patient/family needs and for providing paliative and supportive care to the terminally ill patient/family unit. The Care Center RN utilizes assessment, intervention and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit. The RN / LPN partners with care center manager, the patient, the patient’s family or caregiver, and other staff members on a consistent basis. This position is at the bedside. One patient only.
